We ask that you call and make an appointment before coming in to receive care. Please ask to speak to our triage nurse or medical assistant who can medically assess your needs. The nurse will assess you condition, speak with the doctor if necessary and determine if you need to come to the office or the emergency room.
Our front office staff is available to assist you whenever you call the office for any reason. If you have any concern about the availability for an appointment, accessibility to your doctor, or referrals.
It is essential that you understand your
insurance coverage before you come to the office for your visit. It is important to know what your Insurance Company covers under your plan for the following: Complete Physical Examinations, EKG, GYN visits, Immunizations, and Minor Skin Surgery.
Upon every office visit every patient must present their Insurance Card to the receptionist.
Co-pays must be paid before every office visit.
Scheduling
an Appointment
We value your time and make every effort to be on time for your appointment. It’s important for you to know why we may be running behind schedule and what you can do to help keep our office on time.
When you schedule your appointment,
please tell the receptionist the nature of the visit so she can schedule
appropriately. If you come in with other problems to discuss with the doctor, he or she may ask you to schedule an additional appointment so that you can get the appropriate time necessary for each of your concerns.
If you cannot make your appointment, please call to cancel so we may offer that time to another patient who may be
ill.
Unless we hear from you we reserve that time specifically for you.
We recommend yearly Physical Exams.
Complete Physical Exams require 30 minutes. We only schedule two complete physicals per day. Patients may wait three months for an appointment for a physical. Therefore, if you cannot make our appointment lease call to cancel within 24 hours so that we can offer the time to another patient. If you do not call to cancel your appointment you will be assessed a charge of $25. Occasionally, the doctor may be delayed due to an emergency visit in the office or in the hospital. Feel free to call our office before you leave for your appointment to see if your doctor is running on time.
Medical
Records
One
of the first steps to receiving quality care is
to retrieve your medical records from your
precious healthcare provider. Your doctor needs
your medical history and previous medical
records in order to give you the best care
possible. We suggest that this is done
when you decide to become a patient of Highland
Park Family Practice.
Call your previous doctor's office and request
that your medical records are forwarded to us.
Our office a copy of a medical record release if
needed.
Specialist
Referrals
Patients requesting referral to a specialist
must call the office to make an appointment with
Dr. Miller to obtain a referral. No referrals
will be given without office appointment. We
receive many requests for referrals. Please
allow us at least 3 days to process your
referral.
Do not make an
appointment with the specialist until you have
picked up your paper referral or obtained an
authorization number from our office. Managed
care patients accessing specialty care without
prior authorization will not receive retroactive
approval for non-authorized services.
